A Message From Dr. Reinisch

My name is Dr. John Reinisch. With over 35 years of experience in pediatric reconstructive surgery, I have dedicated my career to treating children with congenital conditions, including giant pigmented birthmarks, known as giant congenital nevus.

As a recognized expert in this field, I have had the privilege of helping hundreds of families from around the world with this condition. My approach is rooted in the belief that early intervention is crucial, and I typically recommend surgery before a child begins to walk.

Early treatment can lead to better outcomes. Young children tolerate surgery better with less psychological trauma. Skin grafting and tissue expansion are easier when children are younger.

If done before children are mobile, there is less likelihood that the child will fall onto the surgical areas, and significantly reduce the long-term impact on your child’s life.

As a parent, you want nothing but the best for your child. When your child is born with a medical condition such as a large, pigmented nevus, it affects not just the child but the entire family.

We understand that each nevus is unique, and the surgical plan is specific for each patient. Depending on size and location, we can use tissue expansion, temporary and permanent full-thickness skin grafts, and have a medical tattoo artist who can restore things like brow asymmetry, if needed.

For example, if a nevus includes the scalp, we use tissue expansion to move the hair-bearing scalp into the proper position to ensure there’s no hair loss. If the nevus involves the area around the eye, we use skin grafts to prevent distortion of the eye.

Your Child's Birthmark

At INI, we recognize that each birthmark case is unique. We use an approach tailored to each individual child—this may include a multi-stage or single procedure.

Types of Birthmarks We Treat

The medical name for a mole is melanocytic nevus. Nevi may develop anywhere in the body and may be caused by cell mutations.

Moles that are present since birth are called congenital nevi. They form during the embryo’s development. About 1 in 100 babies is born with a congenital nevus.

Surgery is the only preferred option for treating large congenital melanocytic nevi.

Some people use “strawberry mark” as a colloquial term for a hemangioma birthmark because of its appearance. Hemangiomas typically develop two months after birth, grow rapidly, stabilize, and remain dormant after years of progressive atrophy. 

Hemangioma, a vascular birthmark, may be treated via traditional surgery or nonsurgical options, like steroids. Depending on size and location, surgical excision may be the best treatment for your child’s hemangioma.

A sebaceous nevus has the following characteristics:

  • Hairless
  • Congenital birthmark 
  • Sometimes looks scaly 
  • Usually found on the head and neck 

This birthmark is typically treated with excision (removal).

Other vascular birthmarks include venous, arteriovenous, and lymphatic malformations. These anomalies vary in size, location, and visibility. Early surgical treatment is often the best treatment plan.
